Default Multiplex Servo Repairs

I have a Multiplex Profi 3 BB Fet servo that has developed a nasty case of the jitters and I would like to send it in for service. Now that Hitec own Multiplex, what address do I use to ship my servo to for service?

Thanks Tres. I have the form printed and I will include that when I send the servo in for a check-up.

How will I know what the repair will cost? Can I assume I'll receive an email with a quote?
