Any of you having belt issues with this truck?

I noticed the front tensioner is installed correctly but still does not perfectly align with the pulley. The tensioner also has a very sharp edge. It seems to snag the belt and cause it to flip onto the top of the tensioner.

I can try tightening it even more I guess.... It was fine for so long until recently.
