After filling out the form
Hit the "Submit by Email" button located on the bottom of the .pdf it opens a addtional window.
Select the second option "Internet Email" this will save the .pdf locally on your PC. (Where ever your files noramlly download.)
Than open your email program.. yahoo/gmail/msn etc. Create a new email to
[email protected] Than just attch the .pdf you saved. Its saves as
LOSB0127-Bulletin.pdf
You will get a auto reply back from Horizon in about 15 minutes.
