Stefan I am sure you are right we pulled apart almost all of the failed plugs because they still looked good even on close inspection. Only when we pulled the wire did it come out easily showing that it was detached inside.
I'm embarrassed to say that I paid more than $9 each at my LHS for them ----- Bad investment on my part.
