I use a chopstick. then grab the crank with a pair of pliers, where the threads are, and gently turn it, just until it pops up a small enough ammount to gently pry it with a flat blade screwdriver, with chopstick removed....

A sleeve puller sounds great. Won't need it often, but when you do, it would be the safest way....
