Every once in a while you'll come onto a defective item, no matter who makes it or how great their reputation is. I'd suggest calling the vendor to see if you can get the defective component addressed, and if not by the seller then hopefully the manufacturer.
Oh, and it never hurts to have a shim behind the collet... Just in case
