Default TA-05 Ball Diff problem

Hey all I got one which has me stumped.

My TA-05 has a Ball diff that keeps coming loose!

It's a 12 ball set up from the IFS-R so it has the TRF416 plates, and I have alloy outdrives and Jaad Ceramic Diff Balls.

Tightens up and feels fine after rebuild but always seems to come loose usually mid-race which is never a good thing.

I have tried replacing the spring, diff plates, screw, lock nut and the little plastic thing the lock nut goes into and I have even checked and re-lubed the 2 * 850 bearings in there, still does it! I have built this the way I was told to which is gradually tighten and work the diff until it's fully tight then back out 1/2 to 3/4 which should be a good setting. The diff never slips initially, but after a run, when you go to tighten it, nothing!

Any ideas? I'm don't think my method of building is off as the other diff in the car has been rock solid.
