What Peded said. Make sure that the tip of the screw is contacting the flat on the motor shaft. Also try using new screws, the tip of the screw should be shaped like a small cup. If the tip is worn, it will back out. Another tip is to use a good quality hex wrench. I'm using an Associated wrench with a hardened tip. It does make a difference.
