Are you running a Keyence 2002 Extreme?

I bought one for my RC12L4 but after I installed it wouldn't work - only to find out it's only rated for 6 cells (no mention of this on the box, only on the inside manual). The LHS wouldn't credit me since I'd cut the wires to length for the car, so I put it in my sedan where it worked just fine.

Are you getting this to work with only 4 cells?
