I use the poor man's solution to this problem. Just flip the wheel nut over so the plastic insert is threaded on first. Actually first thing you do is remove a wheel and thread the nut on normally and make sure it completely cuts threads in the nylon insert. Then remove it and replace the wheel and thread it on insert first. Then you shouldn't have any problems tightening it completely down with a normal wheel wrench. I just use a craftsman one from Sears.
