Thanks! I'll try a set. It got so bad that I ran a 13.5 motor instead just so that I could finish a heat. With the milder motor I didn't have another wheel fly off so I'm thinking it has to do with wheel flex from the additional force of the mod motor and the nut serrations disengaging lap after lap until it finally completely unscrewed.
I had the same thing happen at the Reedy Race this year. It was mostly happening under braking. Once it happens typically the hex on the wheel is shot so you'll need to replace the wheel and definitely go with some other type of lug nut. Also get them on as tight as you can. I ended up going with a locknut.
