When you put the collet on the shaft, does it seem like there isn't much gap at the mating seam? That would do it, stops it from clamping down on the crank. You could file or sand for alittle extra gap.
Hmmm...I don't know. I didn't know to look for that. I'll have to check that out next time. Hopefully, the new collet will hold.
