I have one, but it doesn't completely meet your requirements. It comes the way I purchaed it. It has a module, and a receiver and one set of crystals. I can't find the manual. I have included some pictures. One of them tries to show a very small piece of the plastic that has cracked off. It is extremely small and I didn't notice it until I went to sell this. The radio works perfectly. It is totally inconsequential. I would sell it for $185 shipped in the USA. Payment via paypal.

The middle picture is the one that shows the plastic crack. If you look at the screen, then look at the 6 silver buttons right below the screen. Look at the two center buttons. Right below that you will see a small gap in the plastic. That is the damage.
