Q and A set up sheets
Is there a place or person that has developed a standard question and answer sheet for set ups?
For example
If my car is loose getting into a corner what should I try to help that?
If my car doesn't accelerate as quick out of a corner as I need it too, what can I try to help that?
The list could go on for days, but I think u get the idea.

I've seen something like what you're talking about. Basically a setup guide in a troubleshooting chart format but I forget exactly where it was.
Its not exactly what youre looking for but you might find the Hudy manual helpful though. JQ's setup guide is pretty good too.
