what kind of vehicle is this? Everyones suggestions sound on par with what probably happened. you could of fractured a tooth, and it just happened to get chunked out completely when you refired your engine for that short 3 second period.

I've seen a couple lightweight kyosho flywheels do this same thing.

and you say your shoes were blown? what do you mean, were they just severly worn? or did they actually crumble appart?
