sometimes the copper ring on a glowplug will get caught under a thread, which will make you falsely believe you got it tight. Once it starts running the copper ring moves and now the plug is loose as can be in there and they end up shooting out.

Clean your engine, chances are it inhaled some dirt before it died, make sure the head isnt stripped, and go at it again.
