How do you assemble a slipper wrong? Seriously there is a plate on the inside, the slipper pad, the spur gear, another pad, the other plate. The top shaft aligns it all.
Sometimes during assembly, if one doesn't pay attention, the disk can become dislodged a little bit and not sit where it is supposed to.
